Non-Discrimination Notice

Our commitment to providing care without discrimination.

Kasoya Health PLLC welcomes patients of every background and provides care without discrimination. This Notice describes our non-discrimination commitment and how to obtain language assistance, accessible communication, and to file a concern if you believe you have been treated unfairly.

1. Our commitment

Kasoya Health PLLC does not discriminate against any patient or applicant for care on the basis of race, color, national origin, ancestry, religion, sex, gender identity, gender expression, sexual orientation, age, disability, marital or familial status, pregnancy, parental status, veteran or military status, genetic information, citizenship or immigration status, or primary language. This commitment applies to all aspects of patient care, scheduling, communication, fees, and clinical decision-making.

2. Americans with Disabilities Act (ADA)

We comply with the Americans with Disabilities Act and Section 504 of the Rehabilitation Act. We provide reasonable accommodations for patients with disabilities, including accessible facilities, accessible communication formats on request, and additional appointment time when clinically appropriate.

3. Section 1557 status

Section 1557 of the Affordable Care Act applies to entities receiving federal financial assistance. Kasoya Health is a direct-pay practice and does not currently receive federal financial assistance, so Section 1557 generally does not apply. We nonetheless follow Section 1557's anti-discrimination principles voluntarily.

4. Language access

Dr. Matuk-Villazon is fully bilingual in English and Spanish; care, communication, and written care plans are routinely available in both languages at no additional cost. For other languages, we will arrange interpretation through professional medical interpreters when clinically appropriate; please request this in advance through the contact information below.

5. Auxiliary aids and services

We will, on request, provide auxiliary aids and services for effective communication, such as written materials in larger type, plain-language summaries, or coordination with sign-language interpreters when clinically appropriate.

6. How to raise a concern

If you believe you have experienced discrimination at Kasoya Health, contact us first using the information in section 8. We take every concern seriously and will respond promptly.

7. External complaint mechanisms

You may also file a complaint with: the U.S. Department of Health and Human Services Office for Civil Rights (hhs.gov/ocr); the Texas Workforce Commission Civil Rights Division (twc.texas.gov); the Texas Medical Board (tmb.state.tx.us); or, for incidents in Mexico, the National Council to Prevent Discrimination, CONAPRED (conapred.gob.mx). We will not retaliate for filing a complaint.
